About the team

AccessCare provides community care that supports older people, people with disabilities, and those with complex needs or at risk of homelessness, helping them to live safely and independently at home. Funded by the Commonwealth and Victorian governments, we deliver services across Kingston, Bayside, Glen Eira, Stonnington, and Port Phillip.

About this opportunity:

We’re seeking to employ multiple Team Support Officers ready to make a real difference in people’s lives. In these roles, you’ll provide vital assistance to Care Partners, Client Service Officers, and your teammates, so every consumer receives the high-quality support they deserve. Alongside administrative tasks, you’ll be a friendly point of contact—helping monitor consumer wellbeing and keeping communication flowing between staff, consumers, and families.

Other Responsibilities include:

- Welcoming support and thoughtful service to consumers, carers, and team members
- Ongoing monitoring to help ensure every consumer’s care and safety
- Responsive follow-up to feedback and changing client needs, working with others to find solutions
- Contribute to managing client individual budgets and expenditure
- Help manage incident reporting for continued service improvement
- Assist with Work Health and Safety tasks and planning, always putting wellbeing first
- Foster independence, reablement, and community connection for the people we serve
- Provide backup and hands-on help to other teams, including client home visits when needed

What You’ll Bring:

- Experience supporting people in customer service, community, or care settings
- Warm, compassionate communicator who puts people at ease and builds strong connections




- Great attention to detail, strong organisational skills, and a knack for keeping records accurate and current
- Enthusiasm for teamwork, problem-solving, and helping colleagues achieve the best results
- A commitment to continuous learning, improvement, and supporting a safe and inclusive workplace for all
- Qualifications: Diploma in Health or related field, or experience in a similar role
