Position Description

- Collaborate with the Service Manager and Intake Officer to grow the program by onboarding new Support at Home clients and building your caseload.
Responsibilities
- Build and manage your own caseload of Support at Home clients, ensuring timely assessments and care planning.
- Coordinate and monitor services, liaising with clients, families, and service providers to meet individual needs.
- Maintain accurate client records and reporting in line with organisational and regulatory requirements.
- Travel within the local region to conduct home visits.
About You
- Certificate IV in Ageing Support, Certificate IV in Community Services,
Diploma of Community Services, or Case Management.
- Valid driver's licence.
- Computer literacy.
- Time management and communication skills.
- Knowledge and understanding of home care packages and support at home.
