We’re an innovative and responsive not-for-profit community service provider, helping people across Queensland build stronger and more inclusive communities since 1907.**Your Role as a Case Manager**This is a primarily phone-based role where you’ll help manage the high volume of client referrals currently coming through the service. You will be responsible for:* Engaging with and providing support to families and individuals across the service.* Providing holistic, trauma informed, client-centered case management, support and advocacy for clients.* Facilitate processes to optimize communication, case coordination, access to services and linkages between other service providers, community partners and relevant stakeholders.* Ensuring the needs and outcomes for the target group and objectives are clearly understood by stakeholders and report on barriers being experienced to access service delivery.* Working collaboratively within a team environment, actively participate in regular staff meetings, case reviews and training.* Assist the team in the delivery of activities and programs.This is a fixed-term position ending on the 30/06/2025 with possibility of extension.**What We’re Looking For*** Completed degree in human services or a related field required.* Experience working with families and individuals at risk of homelessness or homeless using trauma informed,



person centred case management principles.* High level communication and interpersonal skills including experience in engaging and working with diverse people with wide-ranging needs.* A commitment to social justice principles, confidentiality and privacy.**Essential Requirements*** Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)* Relevant probity checks required by legislation and WMQ Limited policy.* Current Driver’s Licence and willingness to travel for workWe value diversity, strive for inclusion, and come together to walk alongside those in need to provide hope and compassion.
