About the Role
Your Nursing Agency is seeking an experienced and compassionate Clinical Nurse – Restorative Care Partner & End-of-Life Care Coordinator to coordinate and clinically oversee the Restorative Care Pathway (RCP) and End-of-Life Pathway (EoL) within the Support at Home program. This role is vital in ensuring safe, person-centered care and compliance with Department of Health and Aged Care (DoHAC) requirements.
Key Responsibilities
- Act as the designated Restorative Care Partner and End-of-Life Coordinator
- Work with care partners to ensure clients are correctly supported into these pathways
- Conduct assessments, identify risks, and develop tailored support plans
- Coordinate and oversee allied health, nursing,
and independence supports
- Provide clear communication with families and external providers
- Monitor client progress, goals, comfort, and dignity throughout care
- Ensure compliance with DoHAC requirements, audits, and governance processes
- Deliver pathway-related training and support to Care Partners and staff
- Prepare timely monthly and quarterly compliance and outcomes reports
About you:
- Current AHPRA registration as a Registered Nurse
- 3+ years’ experience in aged care, community nursing, rehabilitation, or palliative care
- Strong knowledge of restorative care, palliative/end-of-life care, and Support at Home reforms
- Proven skills in care planning, risk management, clinical documentation, and compliance reporting
- Experience leading multidisciplinary teams and working closely with families
- Professional, approachable, and confident leadership style
- Strong written, verbal, and interpersonal communication
- High attention to detail and organisational ability
- Empathy and discretion in managing sensitive matters
- Ability to work independently and manage competing priorities
Work Environment & Requirements
- Office-based and community role with regular home visits
- Current driver’s licence and willingness to travel
- National Police Check, NDIS Worker Screening, and Working with Children Check
- COVID-19 and relevant healthcare immunisations
- Participation in clinical on-call roster
