About the Opportunity
We are seeking an experienced Occupational Therapist to join our team at Meadow Springs on a part-time basis. In this role, you’ll be responsible for working in conjunction with a multi-disciplinary team in the delivery of Occupational Therapy services in a person-centred organisation.

Reporting to our General Manager you will be responsible for ensuring a consistently high quality of care is provided to our residents, to facilitate and support their continued enjoyment of life within their level of capability. The role will include, but not limited to:

- Providing high quality Occupational Therapy assessment and intervention for clients within the context of primary health care in a professional, confidential and culturally safe manner, in keeping with accepted best practice standards
- Conducting Occupational Therapy assessments including all areas of activities of daily living, mobility and facility safety
- Ensuring integrated service delivery and follow up,



coordinating and communicating as required with other relevant internal, visiting and external providers
- Providing follow-up monitoring and care as directed through health assessments
- Incorporating a holistic approach to health care that incorporates health promotion activities according to community and organisational priorities
- Working closely with the local community teams and key stakeholders to support regular tracking of progress in monitoring client progress and care, ensuring feedback is provided to staff supporting implementation of changes in response to identified gaps

To be successful, you will have

- Previous experience as an OT, in aged care is desirable
- Knowledge and experience in dementia
- Relevant qualifications and AHPRA registration
- Minimum of 12 months experience as an OT
- Valid working rights in Australia (Passport, Birth Certificate etc)




- Complete a police check
- Ability to communicate effectively with residents, families and all levels of staff
