We're excited to welcome a Credentialled Diabetes Educator Nurse Practitioner (CDE NP) to our Community Connect and Nurse Navigator team at Gympie.
The CDE NP service aims to improve access to specialised diabetes consultancy and patient centred care for geographically isolated and complex patients who do not routinely engage or respond to SCHHS diabetes services within the region. It will provide a responsive service to a high-risk population and those who are disadvantaged by their circumstances.
The CDE NP aims to provide a flexible and responsive service that extends beyond the traditional constructs of inpatient,
outpatients and community models and goes into territories where evidence suggests the demand is high and services have been suboptimal until now such as RACF, supported living homes and carer dependent patients.
Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:
- Understand your broad responsibilities for safety and quality in healthcare, actively participate in continuous quality and safety improvement activities within the workplace and notify a relevant clinical or non-clinical responsible officer when concerns exist about workplace or patient safety.
- Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
- Provide comprehensive patient centred care with advanced clinical and nursing assessment skills, order relevant investigations, and prescribe and implement pharmacological and non-pharmacological therapeutic interventions for patients.
- Create and maintain complimentary partnerships and integration with SCHHS Nurse Navigators, SCHHS Diabetes MDT, General Practitioners, Primary Health Care Teams, Acute Health Teams and Patient Carers.
Remuneration value up to $142 238 p.a., comprising salary rates $75.27 - $78.59 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 8) (Permanent Part-Time position, 60.8 hrs p.f.).
