We are seeking a dedicated andcompassionate Clinical Nurse to join our Renal Services team at theWestern Australian Country Health Service. This role focuses onproviding education and support for clients with Chronic KidneyDisease (CKD) stages 1 to 5 and End Stage Kidney Disease (ESKD),aiming to enhance health outcomes through proactive care andeducation.
As a Clinical Nurse inour Renal Services team, you will:
- Deliver clinical andeducational services to referred patients, ensuringhigh-quality care.
- Consult with inpatientssuffering from CKD and diabetes, developing individualised careplans.
- Collaborate with keystakeholders to provide relevant information and assistance toclients and their families in decision-makingprocesses.
- Participate in preventivekidney health promotion initiatives and work with variousservice providers to implement best-practicestrategies.
- Oversee the planning,implementation, and evaluation of CKD services in the region,contributing to quality care and continuous improvementefforts.
Our ideal candidatewill have the following:
- Experience in ChronicKidney Disease process management and education.
- Demonstrated knowledge inprevention strategies applicable to CKD management.
- A current C or C-A classdriver’s licence.
- Knowledge of EqualEmployment Opportunity and Work Health and Safetyprinciples.
- An understanding andrespect of Aboriginal culture and the ability to effectivelyengage with Aboriginal clients.
KeyResponsibilities:
- Provideindividualised, quality clinical care to patients with CKD andESKD.
- Facilitate appropriateclinical education and support to health service staff,clients, and their families.
- Actively participate inclinical discharge planning and follow-up care.
- Coordinate patient carein collaboration with other health professionals, utilising aproblem-solving approach.
- Monitor and implementnursing practice standards, promoting clinical governancewithin the department.

Country Nursing and MidwiferyIncentive Program:
The Country Nursing & Midwiferyincentive program is available to new and existing nurses andmidwives - across a broad range of patient-facing roles - on fixedterm or permanent contracts at eligible locations. Eligiblenurses and midwives can receive between $5,000 and $17,000 insupport over 12 months.
