We are seeking a dedicated and compassionate Clinical Nurse to join our Renal Services team at the Western Australian Country Health Service. This role focuses on providing education and support for clients with Chronic Kidney Disease (CKD) stages 1 to 5 and End Stage Kidney Disease (ESKD), aiming to enhance health outcomes through proactive care and education.

As a Clinical Nurse in our Renal Services team, you will:

- Deliver clinical and educational services to referred patients, ensuring high-quality care.
- Consult with inpatients suffering from CKD and diabetes, developing individualised care plans.




- Collaborate with key stakeholders to provide relevant information and assistance to clients and their families in decision-making processes.
- Participate in preventive kidney health promotion initiatives and work with various service providers to implement best-practice strategies.
- Oversee the planning, implementation, and evaluation of CKD services in the region, contributing to quality care and continuous improvement efforts.

Our ideal candidate will have the following:
- Experience in Chronic Kidney Disease process management and education.
- Demonstrated knowledge in prevention strategies applicable to CKD management.
- A current C or C-A class driver’s licence.

- An understanding and respect of Aboriginal culture and the ability to effectively engage with Aboriginal clients.

Key Responsibilities:

- Provide individualised, quality clinical care to patients with CKD and ESKD.
- Facilitate appropriate clinical education and support to health service staff, clients, and their families.
- Actively participate in clinical discharge planning and follow-up care.




- Coordinate patient care in collaboration with other health professionals, utilising a problem-solving approach.
- Monitor and implement nursing practice standards, promoting clinical governance within the department.

This position is located at the Kalgoorlie Health Campus. is Australia’s largest Outback city where mining, history and bushland combine, located 595 km east-northeast of Perth.

Country Nursing and Midwifery Incentive Program:

The Country Nursing & Midwifery incentive program is available to new and existing nurses and midwives - across a broad range of patient-facing roles - on fixed term or permanent contracts at eligible locations.



Eligible nurses and midwives can receive between $5,000 and $17,000 in support over 12 months. Please refer to the website for eligibility criteria.

Area Profile:

The Goldfields region of the WA Country Health Service covers the Goldfields-Esperance Region of Western Australia. The region is located in the south eastern corner of Western Australia and covers 770,488 sq km (including offshore islands). The WA Country Health Service Goldfields consists of two main health campuses located in Kalgoorlie & Esperance, and three smaller hospitals located in Laverton, Leonora, and Norseman. The Goldfields is a culturally rich region with a number of Aboriginal communities spread out across the region, 16 distinct Aboriginal language groups and an Aboriginal population that represents 12.3 per cent of our total population, with 1 in 10 people, on average,



identifying as Aboriginal – this in comparison to the state average of 3.6 per cent. The region supports a wide range of industry, including mining, agriculture, aquaculture, and tourism. Mining is the predominant sector in the central and northern parts of the region, with a well-established agricultural sector in the south.
