You will be responsible for:
In this position you will conduct face to face home support assessment with clients daily using a standardized national assessment process, with a focus on wellness and reablement.
Working closely together with clients to de develop individualised support plans to achieve wellness and healthy ageing?goals.
Match and refer clients to appropriate support services.
Ideally service clients in surrounding areas
Contribute to process of continual improvement by always seeking better ways to assist colleagues and clients.
