**About the Role**

As a Social Worker within the palliative care team, you will play a vital role in supporting patients and families facing life-limiting illness. You will provide compassionate counselling and bereavement support, addressing complex psychosocial needs with empathy and professionalism.



Working as part of a skilled interdisciplinary team, your contributions will help ensure holistic and person-centred care that aligns with the values of dignity, respect, and emotional support.

In this role, you will also help shape the delivery of psychosocial services by contributing to the planning, implementation, and evaluation of care strategies. You'll offer specialist guidance and advice across the organisation, and act as a professional resource for colleagues and external agencies. Your expertise will support the ongoing development of best-practice palliative care, making a meaningful difference for patients and their loved ones.

**About You**

You are a passionate and experienced Social Worker who brings a compassionate, client-centred approach to your practice. With a strong foundation in psycho-social assessment and counselling, you are confident working with individuals and families facing complex health challenges. You value collaboration, cultural sensitivity, and are driven to make a meaningful impact in the lives of others.




- Hold an appropriate qualification eligible for full membership with the Australian Association of Social Workers.
- Demonstrated ability to provide expert casework and family counselling in complex care environments.
- Proven experience in delivering psychosocial support and counselling in health or palliative care settings.
- Strong knowledge of social work theories, grief and bereavement support, and culturally responsive care practices.
