**What are we looking for?**
Are you an **experienced NDIS Support Coordinator **looking for your next challenge? Are you passionate about making a difference in the lives of people with disability? If so, we want to hear from you!
As an experienced NDIS Support Coordinator, you will have a comprehensive understanding of the NDIS and the disability sector, as well as a strong track record in providing high-quality support to participants and their families. You will be committed to ensuring that participants receive the support they need to achieve their goals and aspirations and will have a proven ability to work collaboratively with external service providers and stakeholders.
While our office is based in Kelmscott, we offer a flexible work environment that allows you to work from home or other locations, as required. However, travel will be an essential part of this role, as you will be required to attend client meetings, planning meetings, and other appointments across different locations.
**Key Responsibilities**
1. Participant Management
- Work with participants and their families to identify their goals and aspirations.
- Ensure participants are fully informed about their rights, entitlements, and responsibilities under the NDIS.
- Support participants to navigate the NDIS system,
including managing their plan and budget.
2. Coordination of Supports and Services
- Work collaboratively with external service providers and stakeholders to ensure participants receive the support they need.
- Provide guidance and support to participants in selecting and engaging providers.
- Monitor the quality and delivery of services to ensure they meet the participant's needs.
3. Monitoring and Review
- Regularly review participant goals and outcomes to ensure progress is being made towards achieving them.
- Identify any potential issues or barriers that may impact on the participant's ability to achieve their goals.
- Assist participants to prepare for NDIS plan reviews.
4. Record Keeping and Reporting
- Maintain accurate and up-to-date participant records and documentation.
- Ensure compliance with quality standards and contractual requirements.
- Produce regular reports to management and funding bodies as required.
5. Professional Development and Continuous Improvement
- Participate in regular supervision and professional development activities to enhance knowledge and skills.
- Participate in quality assurance and continuous improvement processes to ensure services are delivered to a high standard.
- Keep up-to-date with changes to the NDIS and the disability sector.
**Requirements**:
- A minimum of 2 years' experience in NDIS Support Coordination or a similar role in the disability sector.
- Knowledge and understanding of the NDIS and the disability sector, including relevant legislation and quality standards.
- Excellent communication and interpersonal skills, with the ability to build rapport with participants, families,
and stakeholders.
- Strong organisational skills and attention to detail, with the ability to manage multiple tasks and priorities.
- A current driver's licence and access to a reliable vehicle.
- Valid NDIS Workers Screening Check.
- Working with Children Check.
- Relevant qualifications in disability or related fields are desirable.
