About the Role
668 Care, a nationally registered NDIS provider, is expanding into under-serviced regional NSW—where participants often face significant barriers to consistent, culturally respectful, and high-quality support.
We’re seeking a locally connected leader based in or near Tamworth, Taree, Singleton, Muswellbrook, or the broader New England, Upper Hunter, and Manning regions to drive community engagement, deliver impactful support coordination,
and build trusted referral networks.
You’ll represent 668 Care on the ground—supporting participants, growing referrals, and shaping our local service presence.
Start on a flexible 3-month contractor agreement (ABN) or convert to full-time if you bring an existing caseload.

You’ll represent 668 Care on the ground—supporting participants, growing referrals, and shaping our local service presence.

Your Key ResponsibilitiesParticipant Intake & Onboarding
Build strong referral pipelines with LACs, PHNs, GPs, hospitals, and allied health
Represent 668 Care at community expos, provider forums, and interagency events
Manage compliant onboarding : service agreements, risk assessments, intake forms
Work with our intake and clinical teams for seamless participant transitions
Support Coordination (Level 2 or Specialist)
Manage a diverse caseload across Tamworth, Taree, and Upper Hunter
Implement NDIS plans, crisis manage, and coordinate complex service needs
Navigate SIL, psychosocial,
and thin-market participant challenges
Maintain CRM records, reports, and progress notes to NDIS standards
Community & Stakeholder Engagement
Build and nurture partnerships with local services, NGOs, councils, and referrers
Identify regional service gaps and report them to the leadership team
Represent 668 Care with cultural humility and professionalism in all settings
Who You Are
You might come from :
NDIS Support Coordination, LAC, Allied Health
Aged Care, Mental Health, Youth Work, Justice, or Community Engagement
You bring :
Strong local knowledge and community relationships
Experience working in regional, rural, or diverse settings
Confidence working independently and representing a national provider
Passion for helping participants access quality supports
NSW Driver’s Licence + reliable vehicle (fuel allowance provided)
Comfortable working remotely + travelling regionally
NDIS Worker Screening, Police Check, WWCC (or willingness to obtain)
