Are you able to manage a case load of clients who present with increasing complexity, multiple co-morbidities or disabilities and/or functional limitations-

We are currently looking for an OT who will be responsible for supporting our clients who are Aboriginal and Torres Strait Islander with severe chronic disease and complex needs who are participants of the National Disability Insurance Scheme NDIS and/or clients of the Care Coordination and Supplementary Service Program CCSS.r>

To be successful in this role, you will need to think outside the square,

be supportive and be prepared to deliver a high quality service.You will be a high achiever who will ensure that effective therapeutic services are delivered to each individual customer enabling them recover, develop, improve and adapt.r>

You will be required to actively assist clients to connect with health, clinical and community based support services as outlined in the NDIS plan, understand funding flexibility, be able to successfully link with providers and address any barriers to participation.r>

As you will be dealing with clients, their families, carers and/or guardians, GP's, Allied Health Professionals and suppliers, your communication skills will be of a high level.You will need to develop and maintain collaborative relationships with these key stakeholders that results in effective interactions, minimal service delivery issues and appropriate referral and client management.r>

Our expectations are that you will be patient, highly organised and creative when providing treatments and be able to work in a dynamic and evolving team to achieve optimal client outcomes.ou will need to use a strengths based, client centred approach to identify goals and use a capacity building approach towards achievement and sustainability of these goals.r>

Your aim will be to achieve program objectives, NDIS service objectives and deliverables whilst maintaining your contemporary knowledge of best occupational therapy.r>

By identifying improvement areas for clients and working towards achievement of the clients program objectives, you will not only become a successful member of the team but you will also receive personal gratification of a job well done.r>


Tertiary Qualifications in Occupational Therapy.r>

Current unrestricted AHPRA registration.r>

Strong knowledge, experience and understanding of the role.r>

Able to independently professional knowledge and judgment when performing novel, complex, or critical tasks specific to the occupational therapy discipline.r>

Knowledge and experience of sourcing assistive technology under appropriate funding systems NDIS ENABLE, MAC.r>

Knowledge and experience in utilising appropriate assessments and report writing that match the client's needs.r>

Demonstrated effective interpersonal, oral and written communication skills necessary for good working relationships and client care.r>

Ability to network and develop effective working relationships.r>

Ability to prioritise work, manage time and meet deadlines.r>

Experience in all Microsoft Office programs including Word, Excel, Ou
