Hospital Discharge Liaison Manager (NDIS)

About the Role
Irena Care is a fast-growing, community-based NDIS provider expanding across Townsville and North Queensland. To support smoother transitions for people with disability, we’re hiring a Hospital Discharge Liaison Manager (HDLM) to work directly with local hospitals, aged care centres, and sub-acute services.
This is not a Queensland Health position. You’ll be employed by Irena Care and act as the key link between discharge planners, families, and our NDIS teams—ensuring medically fit individuals receive timely and well-supported access to community services.

- Palm Island
- Aged care centres, sub-acute, and rehab services across the region
Your Purpose
As the Hospital Discharge Liaison Manager, you’ll:
- Identify patients and residents who may be eligible for NDIS
- Engage families, hospital teams, and service coordinators
- Facilitate urgent referrals and smooth care transitions
- Represent Irena Care as the regional bridge between hospital and home
Your Day-to-Day
- Attend MDTs and ward rounds at hospitals or aged care centres
- Lead family and case conferences to secure consent and coordinate next steps
- Prepare SIL, community nursing quotes, AT letters, and Plan Review submissions
- Liaise with OTs, LACs, PHNs, social workers, and hospital discharge planners
- Ensure seamless referrals to Irena Care’s Support Coordination & Community Nursing teams
- Follow up 48h and 7 days post-discharge to ensure service activation
Who We’re Looking For
This role is ideal for professionals with a background in:
- Nursing,
Allied Health, Social Work, or Mental Health
- Aged Care, Rehab, Case Management, or Discharge Planning
You’ll need:
- Minimum 3 years’ experience in health, disability, or case coordination
- Familiarity with hospital or aged care systems and discharge processes
- Understanding of NDIS support categories (Core, Capacity, SIL, SDA, AT)
- Excellent communication and stakeholder engagement skills
- Current QLD Driver’s Licence + reliable vehicle (fuel card provided)
- NDIS Worker Screening + Blue Card (or willingness to obtain)
