About the Role
Irena Care is a nationally registered NDIS provider expanding into thin-market regions across Far-Western NSW, where participants often face significant barriers to consistent, culturally safe, and high-quality NDIS supports.
We’re seeking a community-driven leader based in or connected to the regions of:
- Dubbo Regional LGA
- Broken Hill City
- Far-West NSW — including Wilcannia, Bourke, Brewarrina, and surrounding towns
As our Community Partnerships & Senior Support Coordinator, you will:
- Acquire and onboard NDIS participants through trusted regional relationships
- Deliver Support Coordination for varied, complex needs
- Engage deeply with local services and become the regional face of Irena Care
Start on a flexible 3-month contractor agreement (ABN), or immediately convert to a full-time role if you bring an active caseload.
Work remotely from home, with regional travel and support from a centralised team.
Key Responsibilities
Participant Acquisition & Intake
- Build referral pathways with LACs, PHNs, hospitals, ACCHOs, GPs, and local clinics
- Represent Irena Care at community expos, interagency meetings, and forums
- Lead compliant onboarding: service agreements, risk assessments, and intake
- Collaborate with our clinical and intake teams to support participant transition
Support Coordination (Level 2 or Specialist)
- Manage a caseload of participants across Dubbo, Broken Hill, and remote towns
- Support implementation of NDIS plans, provider engagement, and crisis planning
- Coordinate SIL/psychosocial needs and navigate thin-market barriers
- Maintain CRM documentation,
reports, and service notes to NDIS standards
Community & Stakeholder Engagement
- Cultivate strong relationships with local providers, NGOs, councils, and referrers
- Identify regional service gaps and report solutions to the leadership team
- Represent Irena Care with professionalism and cultural humility
- Be a visible presence in regional disability and health networks
Who You Are
You may come from:
- Support Coordination, NDIS Local Area Coordination, or Allied Health
- Mental Health, Aged Care, Youth Work, Justice, or Remote Community Services
You Bring:
- Deep community knowledge and strong local networks in Western NSW
- Experience working with First Nations, rural, or remote populations
- Ability to work independently and represent an NDIS provider with confidence
- Strong communication, cultural sensitivity,
and commitment to outcomes
Essentials:
- QLD or NSW Driver’s Licence + Reliable vehicle (travel allowance provided)
- Comfortable working remotely and managing travel across vast regions
- NDIS Worker Screening + Police Check + Blue Card (or willingness to obtain)
