Community Partnerships & Senior Support Coordinator

Newcastle, Tamworth & Upper Hunter | Remote Start + Regional Travel

About the Role

Irena Care is a nationally registered NDIS provider expanding into under-serviced regions of Newcastle, Tamworth, Taree, Maitland, and the Upper Hunter, where participants often struggle to access consistent, culturally safe, and high-quality support.

We’re seeking a community-embedded leader to represent Irena Care across the Hunter and New England areas. You will:

- Acquire and onboard NDIS participants through local networks

- Deliver Support Coordination (Level 2 and/or Specialist) for varied and complex needs





- Serve as the trusted local face of Irena Care in community, hospital, and allied health settings

You can begin as a 3-month contractor (ABN) or transition directly to full-time if you bring an active caseload. The role starts remote, with supported outreach across the region.

Key Responsibilities

Participant Acquisition & Intake

- Build referral pipelines with GPs, LACs, PHNs, hospitals, and local providers

- Represent Irena Care at expos, interagency meetings, and forums

- Lead compliant onboarding: service agreements, risk assessments, intake documents

- Liaise with central intake and clinical teams to support transitions

Support Coordination

- Manage a caseload across Newcastle, Tamworth, Taree, and surrounding towns

- Help participants understand and implement their NDIS plans

- Coordinate SIL, psychosocial, and high-needs services

- Maintain accurate reports, notes, and documentation in CRM

Community & Stakeholder Engagement

- Build and nurture relationships with NGOs, councils, clinics, and service providers

- Identify regional service gaps and propose localised solutions





- Represent Irena Care with professionalism, empathy, and cultural humility

Who You Are

You may come from:

- Support Coordination, NDIS LAC, Mental Health, Aged Care, Youth or Justice Services

You Bring:

- Local knowledge and strong community ties in Newcastle, Tamworth, or Hunter

- Experience supporting First Nations, remote, or high-needs populations

- Ability to work independently and build trust in the community

- Cultural sensitivity, reliability, and passion for outcomes

Essentials

- NSW Driver’s Licence + Reliable Vehicle (travel allowance provided)

- Comfortable with hybrid/remote work and regular community travel

- NDIS Worker Screening, Police Check, WWCC (or willingness to obtain)
