Permanent Part Time – 8 hours per fortnight

About the Role

The Transition Care Program (TCP) Case Manager demonstrates advanced clinical knowledge and skills while assuming a management focus and actively works to support staff and nurture a clinical learning environment.

The Case Manager provides coordination and case management to customers of the TCP in a manner that reinforces their rights, independence and dignity and to develop a care plan with an appropriate mix of support services that are integrated, coordinated and responsive to individual needs. Additionally, ensuring that service delivery meets clients’ needs with the TCP guidelines, as per the relevant Quality Standards and the policies and procedures of WDHS.

The successful candidate will demonstrate commitment to person centred care, evidence based best practice standards within scope of practice and ensuring customers and their relatives are involved in decision making relevant to their needs and preferences.

In addition you will deliver:

- APHRA Registered Nurse with at least five years post graduate experience

- Deliver appropriate care to customers, including timely and appropriate discharge planning

- Establish and maintain effective relationships with all key stakeholders to accurately access initial needs of prospective TCP customers

- Be able to liaise with customers to ensure that the appropriate packages of care are aligned with their needs as defined during assessment and care planning processes.

The successful applicant must hold, or be able to obtain, a current National Criminal Police Check, Working with Children Check Clearance and / or NDIS Worker Screening Check (dependent on the position).
