The Access Aged Care team provides co-managed primary care services to Aged Care residents across Australia.
Your New Role
As you step into this exciting onsite part-time opportunity reporting to the Nursing Regional Manager,
you become an integral part of our flourishing organization in a newly created position dedicated to improving the health outcomes of our valued customers.
In this key role, you will spearhead clinical management and coordination efforts, collaborating closely with residential aged-care facilities. We seek an individual with motivation and passion, capable of thriving in an autonomous role that offers both freedom and flexibility. Your base will be in aged care facilities within the Perth region, and you'll frequently travel to partner homes to conduct clinical reviews and manage other responsibilities. While your primary focus will be on Kinross, occasional travel to other areas may be necessary.
The Registered or Enrolled Nurse will collaborate within a multi-disciplinary team, ensuring efficient and high-quality service delivery, backed by the support of the Nursing Regional Manager and the administration team. The successful candidate will also contribute to our care model by engaging in organic business development to expand the resident base. Proficiency in computer usage is a must, and while prior telehealth experience is preferred, it is not mandatory.

Essential Criteria
- Bachelor's or Diploma of Nursing.
- Experience in problem-solving and professional collaboration.
- IT skills and proficiency in documenting customer needs.
- AHPRA Nursing Registration.
- Aged Care experience, leadership preferred.
- Current driver's license and reliable vehicle.

- Organization and time management skills.
- Relationship-building and excellent communication skills.
- Proficiency in computer usage.
